History:

75-year-old male. Symptoms of severe headache and generalized pruritis.

Physical Examination:

Spleen palpable 10 cm. below left costal margin. Liver palpable 3 cm. below the right costal margin. The rest of the exam was within normal limits.

Laboratory Investigations:

1. CBC:(with microscopic differential)

RBC 7.70 x 1012/L

HGB 17.3 g/dL

HCT 54.3 %

MCV 97.1 fL

MCH 32.2 pg

MCHC 33.2 g/dL

RDW 16.4

WBC 18.3 x 109/L

N seg 79 %

N myelocyte 5 %

L 9 %

M 4 %

E 0 %

B 3 %

PLT 484 x 109/L

Morphologic Alterations

Results of the blood smear exam were:

RBC morphology: Essentially normocytic, normochromic

WBC morphology: Within normal limits

PLT morphology: Within normal limits

2. Bone marrow biopsy:

Aspirate differential: Within normal limits

Sections: Markedly hypercellular for age.

3. Cytogenetics:

46, XY. No numerical or structural chromosome abnormalities were detected.

4. Pulmonary function:

Oxygen saturation: 97% (RI 94-100) 

Diagnosis

Polycythemia vera

Clinical Course

The patient is being followed by his local physician and treated with periodic phlebotomy.
